Nutritional Education Waver

Goal:

Atlantic Nutrition empowers individuals to enhance their overall health, vitality, and well-being through personalized nutritional education, guidance on natural foods, and advice on non-medical nutritional supplements. Please note that our coaches provide nutritional information and do not diagnose diseases, disorders, or conditions.

Non-Licensed Educators:

Our coaches are not licensed dietitians, naturopathic doctors, or medical physicians. They provide nutrition education and coaching services, which do not include medical nutrition therapy.

Several states do not require a license for providing non-medical nutritional counseling. New Hampshire is one of these states. Link  to the National Association of Nutrition Professionals (NANP).

Acknowledgment:

To effectively assist through Atlantic Nutrition's education services, information about physical habits, health background, moods, energy levels, lifestyle, and diet may be required. This data is essential for: (1) tailoring nutrition education to your needs, (2) providing valuable knowledge about healthy nutrition, and (3) developing personalized strategies for improved health and vitality. Your information will remain confidential with Atlantic Nutrition’s coaches and will only be shared with consent or if legally mandated.

Health Concerns:

If an Atlantic Nutrition coach identifies potential indicators of a disease, disorder, or condition, the coach will inform the client. This information is not a diagnosis. Prompt consultation with a licensed Physician or Naturopath for a formal evaluation is recommended.

Responsibility to Consult a Physician or Naturopath:

All pertinent details will be shared to ensure safety when seeking nutritional education or information on nutritional supplements for a known disease or condition. It is agreed that a licensed Physician or Naturopath must be consulted before implementing any changes to current treatments.

Truthfulness and Correctness of Information:

The effectiveness of the nutritional education provided by Atlantic Nutrition relies on your commitment to providing accurate and complete information. You are accountable for making decisions about dietary changes and supplement use, and for taking the necessary actions.

Liability Waiver:

By participating in the nutritional education services from Atlantic Nutrition, any and all claims, demands, or causes of action arising from health issues, injuries, or adverse outcomes that may result from participation are released, waived, and discharged from Atlantic Nutrition's and it’s coaches.

Assumption of Risk:

In exchange for being allowed to participate in Atlantic Nutrition's education services, the participant voluntarily assumes all risks, including those of injury or death. The participant waives the right to pursue any claims, lawsuits, or damages against Atlantic Nutrition and it’s coaches related to participation.

Voluntary Participation:

By choosing to participate in Atlantic Nutrition's education services, the participant understands that their involvement is voluntary, and they are solely responsible for their health. The success of the nutritional education is dependent on the information the participant provides and the actions they take. The participant is solely accountable for implementing dietary changes and using supplements. Atlantic Nutrition's coaches will not provide medical advice and will refer clients to other professionals when necessary. The participant releases Atlantic Nutrition and it’s coaches from any liability for loss, injury, or damage incurred through their participation.

By arranging a meeting, the terms outlined above are affirmed to be reviewed and understood, and agreed to. This agreement is a full and final release of all associated liabilities, including any claims, causes of action, costs, or damages related to Atlantic Nutrition's nutrition education services and it’s coaches.
